What you will do:

Assess, plan, and deliver specialist physiotherapy interventions tailored to individual patient goals.
Support patients in developing independence with personal care, domestic tasks, functional mobility, and overcoming cognitive or sensory challenges.
Use a range of therapeutic techniques and adaptive equipment to promote recovery and enhance quality of life.
Manage a complex caseload independently, providing specialist input into treatment plans and discharge decisions.
Complete risk assessments and home environment evaluations to support safe discharge planning.
Liaise with community services, social care, and other agencies to ensure continuity of care.

The ideal candidate will need to be an HCPC registered practitioner and hold a valid UK driving licence for local travel between appointments.
